Community Health Services of Georgia (CHSGa) is a non-profit organization dedicated to providing long-term services and support for older, disabled, and medically complex patients across the state. Our goal is to ensure access to the right assistance as patient needs and goals change over time.
At CHSGa, we offer a range of care services, both medical and non-medical, for extended periods. We have a strong reputation for improving the quality of care and enhancing the consumer experience in home health, hospice, care management, and skilled nursing care.
We are proud to serve every county in Georgia, with a particular focus on rural communities that often face challenges such as poverty and provider shortages. Understanding our population is crucial, which is why we are committed to developing more effective treatments for high-risk and high-cost patients within our system. Quality clinical care, responsive patient navigation and care management, collaboration with partners, and innovative use of technology are at the core of our approach. We believe in engaging informal caregivers as an essential element in supporting older and disabled Georgians, and we continuously seek ways to integrate families into the delivery of care.

Community Health Services of Georgia (CHSGa) is a non-profit organization based in Macon, Georgia. As a 501(c)3, 509(a) (3) corporation, our mission is to support healthcare and related services in Georgia. We achieve this by providing strategic planning, oversight, management, supervision, administrative services, and other essential functions to contracted entities that are also 501(c)(3) organizations.
CHSGa, along with its subsidiaries, offers project management and support services to various healthcare entities. These services include risk management, billing, information technology, and human resources. By partnering with these contracted entities, we aim to enhance the efficiency and effectiveness of their operations, enabling them to better serve the healthcare needs of the community.
Established in 2003, Community Health Services of Georgia operates under the governance of a seven-member board of directors. This dedicated team provides guidance and leadership to ensure that our organization fulfills its mission and upholds our commitment to improving healthcare services in Georgia.
